I, the undersigned, as the parent or legal guardian of the below-named participant, hereby give full consent and approval for my child to participate in dance classes and activities conducted by Shakti Queen Beats LLC.

Acknowledgment and Assumption of Risk

I understand that participation in dance classes and related activities involves physical exertion and carries inherent risks of injury, which may be minor, serious, or even fatal. These risks may be caused by the participant’s own actions or inactions, those of others, or the conditions of the facility.

I understand that these risks may include, but are not limited to: falls, collisions, sprains, broken bones, and other physical injuries. I accept and assume full responsibility for all such risks on behalf of my child.

Facility Safety and Participation Responsibility

I agree to instruct my child to inspect all facilities and equipment before participation and to notify an instructor if any unsafe conditions exist. If at any point either I or my child feel unsafe, we will take all precautions necessary, including refusing to participate.

Medical Consent

In the event of an emergency, I authorize Shakti Queen Beats LLC and its representatives to obtain medical treatment for my child. I understand that I will be notified as soon as possible in the event of such emergency.

Waiver and Release of Liability

In consideration of my child’s participation in these programs, I hereby waive, release, discharge, and covenant not to sue Shakti Queen Beats LLC, its directors, owners, instructors, employees, volunteers, venue partners, and any affiliated entities (collectively referred to as “Releasees”) from any and all liability, claims, or demands arising out of or related to any loss, damage, or injury incurred during participation—whether caused by negligence or otherwise, to the fullest extent permitted by law.

Indemnification

I agree to indemnify and hold harmless Shakti Queen Beats LLC and all Releasees from any claims, including attorney’s fees, made by third parties due to my child’s participation in any classes, events, or activities.

Missed Class and Refund Policy

I understand that:

• Missed classes (for any reason including illness or travel) will not be refunded or pro-rated.

• No refunds will be provided for mid-session withdrawals.

• A “session” is defined as the full set of registered classes over a specific period as outlined in the program schedule.

 

Photographic and Media Consent

I grant Shakti Queen Beats LLC permission to film, photograph, or otherwise record my child during classes or activities. I consent to the use of these images or recordings for promotional, marketing, educational, and artistic purposes across digital and print platforms (e.g., social media, website, flyers).

 

I understand:

• My child’s name may be included in promotional content.

• No royalty, fee, or compensation will be provided for the use of these materials.

• All rights to the content belong to Shakti Queen Beats LLC, and I waive any future claims to ownership or additional consent.

 

Acknowledgment and Signature

I HAVE READ AND UNDERSTOOD THIS ENTIRE WAIVER, INCLUDING ALL ASSUMPTIONS OF RISK, LIABILITY RELEASES, AND INDEMNIFICATIONS. I AM SIGNING THIS FORM VOLUNTARILY, WITHOUT ANY INDUCEMENT. I AGREE THAT THIS DOCUMENT IS A COMPLETE AND UNCONDITIONAL RELEASE OF LIABILITY TO THE FULLEST EXTENT PERMITTED BY LAW.
